Trump actually forgot to sign the executive orders and had no idea what Pence was trying to do, and left the room obliviously.

Trump forgot, Pence made him see the mistake, but he chose to leave anyway, because at that point, he knew he'd look foolish going back.

Trump chose not to do the usual signing for some reason, and Pence didn't understand and foolishly made it look like a problem.

Trump chose not to sign, Pence understood that but acted as though there were a problem to lure the media into writing what can be denounced as fake news.
